There are now options for applying or renewing membership:

1. Apply online. To apply for membership online please click here.
   
2. Renew online. To renew your membership online please click here.
3. Apply by Email: If you want to send your application via email and payment by mail, details and instructions on how to complete the application form are provided below.

• Open the 2022 Online Membership Application available here

• Complete the application by filling in requested information. You can complete the application once you open the form online.

• Right click your mouse button, select the “Print” option, and choose PDF and save the file as a PDF.

• Email fischerh@ameritech.net and add the saved, completed membership file as an attachment. As an alternative, you can also paste the address above into your normal email and attach application and send.

• Mail check to the MC Membership Chairman (please note an overview of the application fees is provided below):

• Complete the application by filling in requested information.

• Right click your mouse button, select the “Print” option, and choose “Print”.

• Mail application form and check to the SMC Membership Chairman (please note an overview of the application fees is provided below):
